MY PREVENTION PLAN / DESIGN EVIDENCE

AI-assisted delivery evidence

14 October 2026. Codex assisted implementation using the explicitly selected feature-dev approach and the supplied FA analysis. Product decisions and clinical hypotheses were not re-derived.

Generation, correction and validation

The AI-generated first review implementation made complete reporting depend on a check-in. During code review this contradicted BR12 and TC14: members can finish reporting while leaving a check-in optional. The corrected domain rule accepts reporting completeness independently, the member service persists this explicit choice, and the UI offers a separate action. TC14 now verifies BR08 for complete reporting without a check-in.

A first Playwright run found that the controlled sharing checkbox immediately reverted while its asynchronous save was pending. The UI now displays the draft checkbox value during the request, keeps success tied to a committed response and falls back to persisted state after failure. A second run found that decorative arrows changed accessible button names; arrows and role glyphs were excluded from accessible labels.

Reset/role switching were also reviewed for retry semantics. A committed control response can be replayed against its original generation/revision only while that response is still the current envelope. Later state changes reject stale work. PostgreSQL integration tests verify both reset and role-switch recovery.

The human user selected the architecture and requested CM-inspired visual direction with additional punch. These are recorded user decisions. The agent performed code review and automated verification; no independent human code-review or native-speaker translation sign-off is claimed. See verification.md for actual final test results.

Provenance and limits

Canonical source, contract and tests are exported from an explicit file allowlist. Environment files, dependency caches, PostgreSQL files and the existing project/.env are excluded. This is evidence of AI-assisted software delivery and correction, not evidence of clinical effectiveness or confirmed CM/MC requirements.
